La realización de un tratamiento endodóntico tiene como finalidad lograr un buen sellado del conducto paraevitar el transporte de contaminantes hacia y desde el conducto radicular, con materiales que presenten en suscaracterísticas la bioco-mpatibilidad con los tejidos adyacentes. A pesar del uso de localizadores apicales, radiografíasy demás elementos auxiliares para un buen diagnóstico y tratamiento por parte del endodoncista, ocasionalmentese presentan situaciones en donde no se respeta el objetivo de mantenimiento de la longitud de trabajo como esel caso de sobre instrumentaciones cuando se intenta retirar instrumentos separados dentro del conducto. Estaspermiten la extrusión de materiales de obturación, principalmente cementos selladores causando complicaciones pos-tratamiento como reacciones inflamatorias, afección de las estructuras vecinas (dolor orbital, daño al dentario inferior, dolor de cabeza, etc.), e incluso, algunos producen infecciones por aspergilosis en los senos maxilares.
Cementos como el AH Plus parecen mostrar resultados favorables aun en casos donde se presenta extrusión depequeñas cantidades al espacio periapical. Los Conductos en C son variantes anatómicas que pueden ser evidenciadas en segundos molares inferiores, que se caracterizan por la unión de los conductos asemejándose la entrada a la letra C. Debido a esta variante anatómica, para el proceso de obturación se debe seleccionar el cemento sellador a utilizar y la técnica para obtener el sellado tridimensional que se desea. La obturación termoplastificada acompañada de onda continua ofrece mejor adaptación al espacio del canal radicular, sobre todo en istmos que son frecuentes en los conductos en C, la técnica de cono único demuestra gran efectividad al ser acompañada de un cemento sellador que tenga como ventaja su capacidad de fluidez para alcanzar irregularidades y que facilite los procesos de reparación a nivel periapical; por mencionar algunas.
